Car Accident/MVA Massage Therapy
Description
In the state of Oregon, your car insurance is legally required to pay for all medical costs associated with your car accident (MVA) - including, in many cases, massage therapy! Main Squeeze Body Lab requires an assessment and massage therapy prescription from a medical doctor, nurse practitioner, chiropractor, or physical therapist before providing services with no out-of-pocket costs. Please have your prescribing provider e-mail their recommendation to anna@mainsqueezebodylab.com , or send a copy or a photo. Alternatively, you may pay our Time of Service rate of $150/hour, with reimbursement when/if billed claims are paid. For liability purposes, we will not provide massage therapy within two weeks of a motor vehicle accident without an assessment. To ensure your full treatment time, please complete your intake form before your arrival. Existing clients, you will need to fill out a new form specific to the incident. For successful billing, ensure your home address, claim number, and point of contact for your claim are accurately listed on your form. Your intake form and all treatment notes will be shared with the insurance company billed for the service, and you may share additional health information unrelated to the accident verbally if you prefer it be kept private. Please note that our cancellation and late policy applies — cancellations/reschedules made within 48 hours of your appointment time will be charged a $25 fee, and full Time of Service price ($150) for same day cancellations and no-shows. We cannot bill for services that were not received. Exceptions may be made in the event of emergency or illness at the provider’s discretion. Please arrive on time to allow your full 60 minutes of table time — lateness may result in shortened service.
